step 1
Claudio's behind tutorial inspired me to share some very cool new features of the clone tool in CS3 accessed through the "clone source" pallet. So to start get yourself an image like the one below mask out an object and set it over a plain background as I have done with the toothpicks. Select you clone tool making sure aligned is checked in the top tool bar and that sample is set to all layers. Now go to >window and open up the clone source pallet.

step 2
Now we are set we need to create our reference point. With the clone tool selected and the toothpicks layer active hold alt down and click at the bottom of the toothpicks to create our source point. (you can actually create 5 in cs3 but thats a whole other tutorial) Now go to the clone source pallet and check the "show overlay" tick box. When you hover over the image now you will see a ghost version of your image follow your cursor. Align the overlay as in my example below and click once to set your reference point. Now undo that using ctrl+z and your reference point is set and you should see the overlay stay where we want it.

step 3
Now we need to make a few adjustments in the clone source pallet and create a new layer to work on beneath our toothpick layer. So in the clone source pallet set the width and height to about 90% and if necessary adjust the offset values to get a good perspective. Create a new empty layer and place it beneath the toothpicks.

step 4
Now with our new empty layer selected and making sure it is beneath our toothpicks layer select a decent sized hard brush and clone in the toothpicks with one continuous brush stroke. When you release your mouse button now you will see that it samples both layers and you are ready to clone in the toothpicks again. Should look like mine below...(ignore my extra layers they were created for my screen shot)

step 5
Now we can clone again, remembering to get it all in one brush stroke,on the same layer to create another pot of toothpicks. you can keep doing this until they fill the image.......
